The Oregon Health Matters Survey is sponsored by the Health Promotion and Chronic Disease Prevention (HPCDP) section of the Oregon Health Authority.

The Oregon Health Authority’s mission is ensuring all people and communities can achieve optimum physical, mental, and social well-being through partnerships, prevention, and access to quality, affordable health care. Data collected from this survey help guide health policy and programming efforts for the State of Oregon.
3) What is the survey about?
This survey asks some general questions about attitudes, beliefs, and behaviors around tobacco, alcohol, marijuana, physical activity, nutrition, and other health related topics.
4) What is the information used for?
Data collected from this survey help guide health policy and programming efforts for the State of Oregon. Most people do find the survey interesting and no identifying information will be connected to the responses you provide.
5) How long will the survey take?
On average it should take approximately 10-15 minutes to complete.
